RN, Case Manager II
Support Department: Case Management
Schedule: Full-Time 8am-4:30pm
Reports To: Director of Case Management
Summary: Assure appropriate admission and level of care placement, plan, design, implement, coordinate and evaluate case across the continuum to facilitate and insure quality, clinical and cost outcomes.
Requirements: Current CA RN license, 2 years Case Management and at least 4 year Acute Care experience in Critical Care or Medical Surgical care, must have working knowledge of Interqual IS/SI, alternative level of care and discharge criteria. Must have current BLS and experience working with computers.
Preferred: Case Management Certificate.
