Jobing Description
The Family Support Counselor I is responsible for implementing a community-based prevention effort for at-risk families. Case management of these families involves assessing families, identifying needs, and assisting families to reach resources within the community. This position also includes in-home training of parenting, home safety skills education, and developmental screening of children.
DUTIES & RESPONSIBILITIES: 1. Prevention Efforts Conduct face-to-face family assessments; recommend specific prevention program Educate families and individuals in home Teach and model parenting skills in home Conduct parent education classes Assist families in identifying/reaching appropriate community resources 2. Case management Conduct a needs assessment Need identified; develop a written plan Link client to community resources, assist in accessing services Provide crisis assistance to arrange for immediate services 2. Documentation Maintain case files and notes, (including releases, etc.) Maintain Work and Activity Sheets, statistics, court reports/letters, as required 3. Community Maintain high profile in the community Maintain/provide current resource referrals within the community Develop/maintain effective working relations with all individuals and agencies impacting program within the community Assist Program Supervisor with developing and implementing community outreach programs Skills / Requirements
REQUIREMENTS:
Bachelor’s degree in Human Services/Behavioral Sciences from an accredited university 1-3 years of successful experience working with families, children and/or parents, and community outreach skills preferred Bilingual English/Spanish is Required Valid CA Driver's License, Automobile Insurance & Reliable Transportation Ability to use diagnostic skills to make assessments Ability to work in a multi-cultural environment Important Notes
