Care Connexxus Adult Day Services, a pioneer in the field of Adult Day Services in California, has served the communities of Riverside and San Bernardino since 1978. Originally known as Cooper-Burkhart House, Care Connexxus began as a volunteer effort of members of Trinity Lutheran Church, who saw the need for a program to assist frail elderly and other mentally or physically impaired adults to remain in the community. Our agency opened in November 1978 becoming the first Adult Day Care Program in the Inland Empire. On July 1, 1980, Care Connexxus (then known as Cooper-Burkhart House) was incorporated as an independent, nonprofit 501(c)(3), charitable organization.

As a Social Worker Assistant, BSW you will work under the supervision of the social worker and will carry out day-to-day tasks, which will include making contact with clients, carrying out routine visits to assess clients needs, keeping up-to-date with changes in legislation, liaising with other agencies and following up inquiries, among other duties not listed. As an Social Work assistant, you will work closely with the MSW in providing front line clinical Social Work Services here at CCI. That will also include Discharge Planning, supportive Counseling for individuals and families, complete Psychosocial Assessments, complete narrative clinical notes, maintain computerized Care Plan documentation. Requirements

Must minimally have A BSW. That is a Bachelor's Degree in Social Work from an accredited School Of Social Work. Experienced preferred but not required. Please note Psychology Majors Need Not Apply

Have oral and written skills in order to prepare and present complete and concise written materials and make presentations to community groups

Must have effective time management skills

Bilingual is a plus!
