Job: RN Case Manager

Here at Chapman Hospice, we believe that our goal is to provide the highest Quality of life possible to the terminally ill patient when the Quantity or duration of life is limited.

We are currently seeking RN Case Managers to join our caring staff. The Case Manager is an experienced Registered Nurse who provides skilled palliative and end of life care in the community setting. The Case Manager facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination and medical management of hospice services for patients and their significant others.

Must have current California RN license

SNF experience preferred

Ability to recognize the special needs of Hospice patients and others in the home

Ability to communicate with patients and significant others in an effective, mature, caring manner

Ability to maintain harmonious constructive relationships with internal and external customers
