Jobing Description
Here at Chapman Hospice, we believe that our goal is to provide the highest Quality of life possible to the terminally ill patient when the Quantity or duration of life is limited.

We are currently seeking a Hospice Director of Patient Care to join our team. As a Hospice Director of Patient Care, you will be directly responsible for implementing and supervising the day to day management of Hospice patient care services.
  • Performs any and all professional duties
  • Ensures that the nursing process will be utilized to maximize care and attain patient goals
  • Has knowledge of Hospice Medicare/Medicaid guidelines
  • Has received training in Hospice Care Delivery
  • Supervises all licensed personnel
  • Abides by department policies and procedures

    Duties/Responsibilities
    Performs the following duties:
  • Gives and receives reports
  • Orientation and instruction of other licensed nursing personnel
  • Assesses patients' needs, condition and care
  • Assists staff to develop Hospice Plan of Care
  • Reviews and transcribes physician orders
  • Reviews patient's medical and social history
  • Periodically reviews Hospice PLan of Care with nursing staff and IDG
  • Evaluates licensed nurses upon completion of introductory period and annually or as needed thereafter
  • Supervises licensed nurses a minimum of 1 day/wk in the field
  • Assists with nursing on call as needed
  • Assists with/or completes Hospice admissions as needed
  • Prepares and facilitates Hospice In-Services including preparation of yearly calendar for licensed nursing personnel
  • Function as RN Case Manager needed
  • completes competency testing of licensed nursing personnel at time of hire and yearly thereafter
  • Other duties as requested by supervisor

    Working Conditions
  • Well lighted, well ventilated rooms and halls
  • Sits, stands and walks most of work day
  • Exposed to infection and odors
  • Must deal with patients and public on personal basis under any conditions
  • Must handle emergency situations, give emergency care and give reassurance to patients/families and staff


  • Skills / Requirements
  • Graduate of an approved school of nursing
  • Current licensure in the State of California
  • Previous experience with geriatric patients desirable
  • Minimum of 2 years current management experience preferred, may include SNF and/or Hospice
  • Must have sincere desire to work with the aged and the terminally ill
  • Must have thorough knowledge of medical, social and psychological needs of patients in this type of environment
  • Msut be able to supervise and instruct others
  • Must be well groomed, courteous, tactful, patient, kind and pleasant when dealing with patients, staff and families
  • Must be level headed in emergency situations
  • Must have acute sense of responsibility
  • Must possess a spirit of cooperation and enthusiasm
  • Membership in a professional organization preferred
  • Must maintain education in the field of management
