Our 367 bed acute tertiary care facility is fully accredited by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO), the nation's oldest and largest hospital accreditation agency. We are located in Palm Springs, California, historically renowned as a legendary celebrity getaway and resort community. The dramatic desert environment offers spectacular views of the beautifully landscaped gardens, royal palms, majestic mountains and clear blue skies. Palm Springs and the fast-growing surrounding areas offer a variety of well-known restaurants, shops, cultural activities such as theater, museums and film festivals, and outdoor recreation including championship golf, tennis, hiking and horseback riding. The ICON Clinical Care Coordinator (CCC) is an experienced professional nurse who through education, clinical direction, staff development, consultation, research and monitoring various patient outcomes, works towards achieving the goals of ICON and Desert Regional Medical Center. The ICON CCC will have oversight and coordination responsibilities over clinical care coordination,patient education and outreach programs affiliated with ICON's Stroke and Spine Programs. The CCC will assist with strategies for obtaining JCAHO Disease Specific Care Certification for related ICON programs, in collaboration with appropriate hospital inpatient departments. The ICON CCC will work with ICON physicians to collect and maintain outcome data, as well as initiate research protocols as appropriate. All activities will be in compliance with the hospital's performance improvement plans, JCAHO, and other licensing, accrediting and regulatory agencies. All access to Patient Health information will be for treatment and or data management purposes. Qualifications : Graduate of a School of Nursing with a Bachelor's Degree in Nursing. Master's preferred. Current CA License as a Registered Nurse. Minimum of three (3) years professional experience in Orthopedics, Neurosciences, or related clinical area, with minimum of two (2) years experience working directly with patients and families, education capacity preferred. Ability to communicate effectively with peers, physicians, administrative staff and others. Excellent interpersonal skills; written and verbal communication skills; computer skills. Flexibility; willingness to work evenings/weekends as required by classes/support groups We Value Diversity!
